">The salary for this role is approximately £35,000 - £40,000 per annum, depending on experience. As a member of our team, you'll benefit from an excellent pension scheme, staff discounts, immediate life cover, and ill-health protection.
">About the Role
">We're seeking a highly skilled Community Support Specialist to join our Community Integrated Response Team (CIRT). You'll work collaboratively across a multi-disciplinary team, providing urgent assessments, care, and treatment in people's own homes to prevent hospital admissions.
">Key Responsibilities
">- You will be involved in supporting the delivery of Personalisation and inclusion Pathways for Adults and Carers, ensuring compliance with the Care Act 2014.
- You will use the 3 Conversations model (3Cs) to identify individuals' strengths and aspirations, maximising their independence and wellbeing.
Working Hours and Location
">The successful candidate will work 36 hours per week, with various shift patterns to cover 8am-8pm service, 7 days a week, including weekends (9-5pm on a rota basis).
