Key Responsibilities

• Giving unpaid carers the breaks they need by providing interim support to those they care for.
• Offering emergency response assistance to carers in crisis.
• Supporting hospital discharge services and helping prevent avoidable admissions.
• Working alongside our multidisciplinary teams to provide person-centred care.
• Collaborating with healthcare professionals to ensure seamless transitions of care.
