Key Responsibilities
- Giving unpaid carers the breaks they need by providing interim support to those they care for.
- Offering emergency response assistance to carers in crisis.
- Supporting hospital discharge services and helping prevent avoidable admissions.
- Working alongside our multidisciplinary teams to provide person-centred care.
- Collaborating with healthcare professionals to ensure seamless transitions of care.
