Job Summary

We are seeking a compassionate and skilled Support Worker to join our team. As a Support Worker, you will play a vital role in providing individualized support to our clients, enabling them to achieve their goals and live fulfilling lives.

Key Responsibilities

  • Provide person-centered support to clients with physical, intellectual, sensory, and psychological disabilities
  • Assist clients with daily living activities, such as personal care, meal preparation, and household tasks
  • Support clients to participate in community activities, social events, and recreational programs
  • Collaborate with the care team to develop and implement individualized support plans
  • Monitor and report on client progress, identifying areas for improvement and implementing changes as needed

Requirements

  • Certificate IV in Disability, Community Service, or Aged Care (or equivalent)
  • NDIS Worker Screening Check (or willing to complete)
  • National Police Check
  • Working with Children Check
  • First Aid certificate and CPR (or equivalent)
  • NDIS Workers Orientation module (or willing to complete)
  • Eligibility to work in Australia
  • Fully COVID Vaccinated (Triple vaccinated) with evidence
  • Previous experience supporting people with disabilities
  • Demonstrated ability to work with minimal supervision and collaboratively as a member of a multidisciplinary team
  • Demonstrated commitment to professional development, quality improvement, and evidence-based practice
