Key Responsibilities:
- Triage GP referrals and coordinate weekly clinics to ensure seamless patient care.
- Liaise with healthcare professionals to monitor patient health issues, including obesity, diabetes, epilepsy, and asthma.
- Conduct regular health reviews and ECGs to maintain accurate and detailed medical records.
- Run vaccination clinics for patients and staff to promote health and well-being.
- Develop audits and implement action plans to improve patient care and outcomes.
- Contribute to the induction of new team members to ensure a smooth transition.
Requirements:
- Registered Nurse Qualification (RGN)
- Valid NMC Pin
- Clear DBS
- Proven track record of delivering person-centered care, including care planning
