Practice Nurse

We are seeking an experienced Practice Nurse to join our team at Evergood Associates. As a Practice Nurse, you will play a vital role in the delivery of high-quality patient care, focusing on chronic disease management and prevention.

• Implement evidence-based practice for the effective management of patients with long-term conditions, including care for elderly and housebound patients.
• Work as a senior member of the practice multidisciplinary team to ensure nursing services are delivered effectively to the entitled patient population.
• Support the Infection Prevention Control (IPC) lead in implementing audits, training, and evidence of compliance to the practice manager.
• Develop and implement an effective care management programme for frail and housebound patients registered at the practice.
• Assess patient needs and provide healthcare that is appropriate, incorporating evidence-based practice.
• Develop and implement health promotion and wellbeing programmes.
• Implement and evaluate individual treatment plans for chronic disease patients that promote health and wellbeing.
• Identify, manage, and support patients at risk of developing long-term conditions, preventing adverse effects to patients' health.

• Registered Nurse with the Nursing and Midwifery Council.
• Experience in chronic disease management, including diabetes, hypertension, and respiratory conditions.
• Previous Practice Nurse experience.
• Qualified smear taker.
• Imms/Vacs trained, including childhood imms.
