About the Role

We are seeking a compassionate and dedicated Care Worker to join our team at The Hospice of St Francis. As a Care Worker, you will play a vital role in providing high-quality care to patients in their own homes, supporting them to live independently and maintain their dignity.

Key Responsibilities
  • Deliver personalized care to patients in their own homes, ensuring their physical, emotional, and social needs are met.
  • Work collaboratively with the care team to develop and implement individualized care plans that meet the unique needs of each patient.
  • Provide emotional support and companionship to patients and their families, promoting a sense of comfort and well-being.
  • Assist patients with daily living activities, such as bathing, dressing, and medication management.
  • Monitor patients' conditions and report any changes or concerns to the care team.
  • Participate in ongoing training and professional development to enhance your skills and knowledge in end-of-life care.
Requirements
  • Care Certificate or NVQ Level 2 qualification in Health and Social Care.
  • At least 3 years of experience in a similar role, with a proven track record of delivering high-quality care.
  • Current driving license and access to a car.
  • Ability to work independently and as part of a team, with excellent communication and interpersonal skills.
  • Knowledge of and experience in care at the end of life and after death.
