You will be joining a forward-thinking Primary Care Network (PCN) that is committed to providing high-quality, patient-centered care.
The team includes a diverse group of healthcare professionals, including Clinical Pharmacists, Paramedics, Physician Associates, and Social Prescribing Link Workers, who work together to enhance patient care.
In this role, you will have the opportunity to work collaboratively with the team to deliver high-quality care and contribute to clinical innovation and service development.
Salary: £24,000 per annum (pro-rata for part-time), 4 sessions per week
