We are seeking an experienced Clinical Pharmacist to join our team, working in partnership with doctors, nurses, and other healthcare professionals to provide excellent patient care.


This role will involve managing the repeat prescribing reauthorisation process, reviewing patient requests for repeat prescriptions, and monitoring medicines reaching review dates. You will also work with care home staff to improve safety of medicines ordering and administration.


The successful candidate will have a strong understanding of general practice and pharmacy principles, as well as experience in providing and using information to support the delivery and performance management of services.


You will be required to communicate complex and sensitive information in an understandable form to a variety of audiences, including patients, carers, and healthcare professionals.

Key Responsibilities:



  • Manage the repeat prescribing reauthorisation process, reviewing patient requests for repeat prescriptions and monitoring medicines reaching review dates.
  • Work with care home staff to improve safety of medicines ordering and administration.
  • Provide a telephone help line for patients with questions, queries and concerns about their medicines.
  • Undertake clinical medication reviews with patients and produce recommendations for the GP on prescribing and monitoring.
  • Contribute to reductions in medicine-related hospital admissions and readmissions by supporting patients to get the best outcomes from their medicines.

Requirements:



  • Completion of an undergraduate degree in Pharmacy, and registration with the General Pharmaceutical Council.
  • Completed the CPPE primary care pathway or be working towards it.
  • Understanding of the work of General Practice.
  • Drive and determination to promote and support the role of pharmacy staff in General Practice.
  • Able to work under pressure and to meet deadlines.
  • Work effectively independently and as a team member.
  • Understands the principles of equality and diversity and commitment to addressing health inequalities and patient empowerment.
  • An Enhanced DBS check is required for the role.
