Job Title: Nurse Assessor
Job Summary:
Gray Healthcare is seeking a skilled and experienced Nurse Assessor to join our team. As a key member of our assessment and support team, you will be responsible for conducting clinical assessments and developing individualized support packages for individuals with complex needs.
Key Responsibilities:
- Conduct comprehensive clinical assessments to identify individual needs and develop support plans
- Work closely with the Assessment and Contracts Manager to ensure support packages are deliverable and meet the needs of individuals
- Review reports and evidence outcome measures to inform clinical practice and business performance
- Identify and mitigate complex risks for a community setting
- Produce clinically informed assessments with robust risk assessment and identifying risk mitigation measures
- Attend internal and external MDT meetings to inform assessment and risk mitigation
- Build professional working relationships with stakeholders and other external professionals
Requirements:
- Registered Mental Health Nurse or Occupational Therapist
- Experience of assessing and working with individuals with complex needs, including mental health, learning disability, ABI, personality disorder, autism, and forensic backgrounds
- Understanding of continuing healthcare and transition of individuals from hospital/residential settings into the community
- Applied knowledge of the Mental Health Act, CTO's, and the Mental Capacity Act, Deprivation of Liberty Safeguards, and Best Interest Decision making
- Expertise in assessment and management of risk, with strong report writing skills and computer literacy
- Full UK Driving Licence and access to a vehicle
