Job Title: 1st Shift Weekends LPN

At Giving Home Health Care, we are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team in the Alton, MO area. As a LPN, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.

Responsibilities:
  • Administer patient care in accordance with a physician-established care plan.
  • Complete compliant documentation of the care provided in the system of record.
  • Manage and administer prescribed medication, treatment, and therapies.
  • Conduct patient assessments, coordination of care.
  • Monitor vitals and GI intake and output.
  • Assess skin integrity and administer wound care.
  • Monitor for changes in the patient's condition such as weight loss/gain, self-care abilities, and indicators of disease progression.
  • Communicate with family members, physicians, case managers, and other prescribed individuals regarding the health of the patients.
  • Provide education and training to the patient, their family, and/or the Home Health Aide.
  • Collaborate with and supervise Certified Nursing Assistants and Personal Care Attendants.
  • Perform other personal care services as necessary to meet the patient's needs.
Requirements:
  • Graduate of an accredited college or university, Associate or Baccalaureate School of Nursing.
  • Hand-eye coordination and manual dexterity.
  • Ability to provide basic medical management up to medically fragile end of life care.
Preferred Qualifications:
  • 1+ years of experience as an RN/LPN in a clinical or home health setting.

Compensation: $35 - $50 an hour. Schedule: Part-time, 2-4 hour visits per month. Flexible schedule to accommodate patient needs.
