Our CAMHS/CALDS Children's Continuing Care Team requires a highly skilled individual to fill a fixed-term vacancy. As a Care Manager, you will play a crucial role in ensuring the delivery of integrated services or packages of care for patients/service users who meet children's continuing care criteria.

Your responsibilities will include:

  • Designing and implementing effective care plans that address the unique needs of each patient/service user.
  • Providing clinical guidance and support to Care Coordinators to ensure compliance with regulations and best practices.
  • Evaluating commissioning decisions to ensure alignment with national priorities and local service strategies.
  • Collaborating with Care Coordinators to develop new referrals for Children's Continuing Care applications, including identifying suitable providers and packages of care.
  • Conducting quality assurance reviews of Children's Continuing Care applications for patients/service users on colleagues' caseloads.
  • Presenting cases at the Quality Assurance/Peer Review Panel and following up on actions as necessary.
  • Communicating ongoing concerns about provider performance and updates on action plans.
  • Reviewing cases to ensure that contractual arrangements are being met.
