We are seeking a skilled and compassionate Lead Support Worker to join our team in Westbury-on-Severn. As a Lead Support Worker, you will provide exceptional day-to-day support to individuals with complex needs, empowering them to overcome barriers and participate fully in their community.
Key Responsibilities
- Support individuals in all aspects of their daily life, enabling their fullest participation in the life of their community.
- Develop and implement support plans to achieve personal outcomes.
- Liaise with external professionals to devise risk assessments and support plans.
- Communicate specialist knowledge to colleagues to produce a coherent and integrated package of support.

Requirements
- Patient, calm, and supportive with good observational and listening skills.

- Diploma in Health and Social Care Level 2 or equivalent (Level 3 desirable).
