Bush & Co is a renowned organisation in the field of case management and rehabilitation. We are seeking a highly skilled Clinical Case Manager to join our dynamic team.
Role Overview
The successful candidate will be responsible for managing a caseload of clients, leading on assessments, and signposting and advocating for their needs. They will also be responsible for managing administration, including sending instructions to third parties and writing and reviewing reports.
Key Responsibilities
- Manage a caseload of clients through their rehabilitation
- Lead on assessments in the home and rehabilitation and goal setting
- Signpost and advocate for what clients need, and assess what needs to be in place for them to live a fulfilled life
- Manage all administration in relation to cases, including sending instructions to third parties and writing and reviewing reports
Requirements
- At least 5 years clinical experience
- A valid clinical registration with an appropriate clinical body
- Previous experience establishing or implementing rehabilitation packages
- Previous experience within case management or working in the community setting is advantageous
- Strong relationship building skills
- Driver with access to own vehicle
- Willingness to travel extensively across the UK, Scotland, and Wales
