About the Role

We're seeking a Support Practitioner to join our team at Cornerstone Community Care. As a Support Practitioner, you'll work with individuals with various support needs, providing assistance with daily living tasks, personal care, and medication management.

Key Responsibilities

  • Support individuals with daily living tasks, including cooking, personal care, and medication management
  • Assist with hobbies and activities, such as karaoke, arts and crafts, and swimming
  • Develop and maintain positive relationships with service users and their families
  • Work collaboratively with colleagues to provide a high-quality service
  • Participate in training and development opportunities to enhance skills and knowledge

Requirements

  • Good communication and interpersonal skills
  • A caring and empathetic approach
  • Ability to work flexibly, including evenings, weekends, and public holidays
  • Confidence in using computers and other technology
