Job Overview

This is a fixed-term/secondment position for several months due to maternity cover.

Applicants for the secondment position must obtain permission from their current line manager prior to applying for this post.

The Cancer Navigator will work alongside the Cancer Clinical Nurse Specialist to enhance the care experience for individuals with cancer. They will collaborate within the multidisciplinary team to provide support, handle non-complex tasks, and enable registered practitioners to focus on managing patients' complex care needs.

The Cancer Navigator will carry out interventions within agreed parameters and defined protocols of care under the delegation of registered nurses within the speciality.

Main Duties of the Job

Practise in accordance with Board of Health standards and statutory requirements, operate within the boundaries of the role, and assess competencies.

Work proactively as a member of the multidisciplinary and multi-professional healthcare team.

Manage a defined caseload of patients under the supervision of the CCNS. Conduct assessments and interventions as directed by the CCNS.

Assist in identifying patients' health needs within the scope of eligibility. Cover the planning, organisation and delivery of clinical care as agreed with the CCNS, in accordance with clinical governance and within agreed professional standards and guidelines.

Conduct reviews and evaluate the care provided under the indirect supervision of a Registered Nurse and make suggestions to the registered nurse regarding modifications to the patient care plan.

Discuss and make suggestions for adjustments to individual care plans and their health and wellbeing needs with the care team and understand their own role in delivering care to meet those needs.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
