Job Summary

We are seeking an experienced Community Nursing Sister/Charge Nurse to join our team within the Wokingham Community Nursing Services. The successful candidate will be responsible for leading a team of nurses to provide high-quality care to patients in their own homes.

Key Responsibilities
  • Manage a complex and diverse caseload, working to prevent admission to the acute sector and supporting patients in their preferred place of care.
  • Maintain detailed, accurate, and confidential patient records, adhering to Trust policies.
  • Lead an effective supportive team, working together to provide clinical advice and support to other staff and patients.
  • Manage a defined caseload of patients, with continuing responsibility for the assessment of care needs, the development, implementation, and evaluation of care programs.
  • Accountable for the standards of care and for determining the activity of your team.

Requirements
  • Adult nursing qualification along with NMC registration.
