About the Role

We are seeking a highly skilled and dedicated Mental Health Practitioner to join our Community Assessment Team. As a key member of our team, you will be responsible for providing high-quality assessment and treatment services to children and young people experiencing mental health issues.

Key Responsibilities
  • Conduct initial assessments for children and young people referred to our service, ensuring safeguarding and risk management are implemented to the highest standard.
  • Develop and implement care plans that meet the individual needs of each child or young person, working closely with families and other professionals as needed.
  • Participate in multi-disciplinary case formulation and contribute to the development of treatment plans.
  • Deliver group interventions and work collaboratively with colleagues to provide evidence-based care.
  • Represent our service in professional forums and complex networks, ensuring effective communication and collaboration with other agencies.
