About the Role

We are seeking a highly skilled and compassionate Hospice Care Specialist to join our team at Saint Francis Hospice. This is an excellent opportunity for a motivated individual to make a real difference in the lives of our patients and their families.

Job Summary

This role involves providing high-quality care and support to individuals with life-limiting illnesses, ensuring that their physical, emotional, and spiritual needs are met. You will work collaboratively with our multidisciplinary team to develop and implement personalized care plans, promoting comfort, dignity, and well-being.

Key Responsibilities
  • Provide expert care and support to patients with complex needs
  • Develop and implement effective care plans, collaborating with the healthcare team
  • Conduct comprehensive assessments, identifying areas for improvement
  • Maintain accurate records and documentation
  • Participate in ongoing education and training to enhance skills and knowledge
Requirements
  • Registered Nurse (Level 1) qualification
  • ENB 998 or equivalent
  • Excellent communication and interpersonal skills
  • Able to work autonomously and as part of a team
  • Flexibility to work varying shifts, including nights
