We are seeking a skilled Geriatric Practitioner to join our dynamic team at East and North Hertfordshire NHS Trust. The successful candidate will be responsible for providing high-quality care to vulnerable patients, working closely with multidisciplinary teams to ensure comprehensive assessments and effective management plans.
The ideal candidate will have a keen interest in frailty care, a strong understanding of the Comprehensive Geriatric Assessment (CGA) process, and excellent communication skills. Experience in working with complex patients and a willingness to undertake research and audits to further develop the service are essential.

Main DutiesThe successful candidate will be responsible for:
- Providing nurse-led assessments to support ward staff in effectively managing complex patients
- Acting as an advocate for patients, providing education to the wider trust on effective management of frailty, strategies to reduce deconditioning, and education around clinical frailty scoring
- Supporting pilot activity and contributing to the development of new pathways for frailty care
- Working collaboratively with multidisciplinary teams to ensure seamless transfer of information between acute and primary care services
