We are seeking a highly skilled and experienced Nurse Case Manager to join our team at Leaders In Care Recruitment Ltd. As a Community Nurse Case Manager, you will play a vital role in supporting clients with complex care needs in the North West region.
Key Responsibilities- Conduct comprehensive assessments in the community to identify client needs and develop personalized care plans.
- Train and assess the competencies of support workers to ensure they are equipped to provide high-quality care.
- Collaborate with the multidisciplinary team to ensure seamless care delivery and effective communication.
- Attend meetings and make referrals as necessary to ensure clients receive the best possible care.
- Hold regular team meetings to discuss client progress, address concerns, and provide guidance and support.
- Manage a team of non-clinical case managers to ensure effective care delivery and team performance.
- Registered General Nurse (RGN) or Registered Sick Children's Nurse (RSCN) qualification with a strong background in complex care.
- Experience in tracheostomy and complex care management, including peg feeds and ventilator management.
- Valid driver's license and ability to travel around the North West region.
- Experience caring for both adults and children with complex care needs.
- Annual salary of £43,000.
- £3,000 car allowance.
- Mileage expenses.
- Full-time hours: 37.5 hours per week, Monday to Friday.
