Diabetes Specialist Nurse Primary Care Professional

Connaught Resourcing is partnering with a leading National Service provider to recruit permanent Diabetes Specialist Nurses for primary care positions across the country.

This is a hybrid-working opportunity where you will conduct various clinics within community settings and work from home in between.

As a proactive Diabetes Specialist Nurse, you will identify sub-optimally controlled patients with Type 2 diabetes, optimize their oral therapy, conduct point-of-care testing, and refer for specialist insulin treatment when necessary.

Key responsibilities include:

  • Diabetes diploma or equivalent
  • A minimum of 3 years experience managing diabetes within primary care
  • Knowledgeable and confident in providing education, support, and advice to people with Type 2 Diabetes
  • Skills and knowledge in optimizing oral hypoglycaemics
  • Full driving license
  • Current NMC registration
  • IT literacy, including primary care systems
