Family Practitioner Role Overview
In this role, you will provide individual support for parents and young people, exploring their wellbeing goals and providing advice on whole family wellbeing strategies to promote improvements in emotional health and wellbeing.
You will work closely with families to understand their needs and develop tailored support plans, linking in with other services as necessary to ensure they receive the right support at the right time.
As a key member of the Platfform team, you will contribute to ongoing service improvements and make the most of supervision, reflective practice, personal development, and new ideas to enhance your practice and support outcomes.
Key Responsibilities:
- Provide individual support for parents and young people to promote emotional health and wellbeing
- Develop and implement tailored support plans to meet the needs of families
- Work closely with other services to ensure families receive the right support at the right time
- Contribute to ongoing service improvements and professional development
