About the Role

We are seeking a skilled and compassionate Nurse Associate to join our team at Sirona care & health CIC. As a key member of our healthcare team, you will play a vital role in delivering high-quality patient care and support.

Key Responsibilities
  • Conduct holistic assessments and implement care plans for patients with acute and chronic conditions.
  • Monitor patients' health needs and well-being, assessing their condition and providing appropriate interventions.
  • Use clinical reasoning skills to assess, plan, implement, and evaluate patient-centered interventions.
  • Collaborate with patients, carers, and family members to identify achievable goals and develop personalized care plans.
  • Contribute to the facilitation of referrals to other services and work within a multi-disciplinary team to provide integrated care.
  • Assess and arrange the provision of equipment and provide a safe and comfortable environment for patients and carers.
  • Support and educate patients and carers to promote independence, good health, and personal safety.
  • Maintain accurate and informative records of interventions in accordance with policy.
  • Participate in training and development opportunities to enhance your skills and knowledge.
