Falkirk Council is seeking a highly skilled and motivated Senior Home First Practitioner to join their dynamic Home First Team. This is an exciting opportunity to be part of a busy and innovative service that is dedicated to delivering high-quality care and support to the community.
Key Responsibilities:
- Lead and manage a team of practitioners to deliver effective and efficient care services.
- Develop and implement strategies to improve service delivery and outcomes.
- Collaborate with other teams and stakeholders to ensure seamless service provision.
- Provide supervision and guidance to team members to ensure they have the necessary skills and knowledge to perform their roles effectively.
- Contribute to the development of policies and procedures to ensure compliance with regulatory requirements.
Requirements:
- SVQ Level 3 in social care or equivalent qualification.
- Excellent numeracy and literacy skills.
- Knowledge and experience of local authority services and data protection legislative requirements.
- Knowledge and understanding of the Home First ethos.
- Experience in a health and/or social care setting with excellent leadership and management skills.
- Excellent communication, supervision, and IT skills.
- Ability to work to deadlines within a busy team.
