Primary Care Team Coordinator

We are seeking a highly motivated and organized individual to join our Primary Care Team as a Patient Services Care Coordinator. In this role, you will be responsible for managing administrative duties, registering patients, booking appointments, and ensuring all information is properly managed and followed up.
Main Responsibilities- Respond to patient queries and liaise with the wider primary care team.
- Manage appointment requests and signpost patients to our range of services.
- Maintain patient records and ensure confidentiality.
- Utilize information systems to support efficient workflow processes.
- Email, scan, and code clinical correspondence.
- Process prescription requests in a timely manner.
- Support patient health promotion recall campaigns.
- Encourage uptake of our digital tools to support patient self-care.
- Participate in mandatory training, annual appraisal, and continued education.
- Maintain the Health and Safety of all staff, patients, and visitors.
- Ensure all patients and visitors at reception and telephone callers to the practice are greeted professionally and politely.
- Answer all incoming phone calls from patients and other healthcare professionals and act accordingly.
- Deal with administrative queries and requests from patients and act as liaison with the clinicians as necessary.
- Book, cancel, and rearrange appointments via the clinical system.
- Support and assist clinicians as required.
- Ensure prescriptions are handled accurately and efficiently following protocol.
- Deal with GP Links, including amendments and deductions of patient records.
- Monitor incoming electronic test results to ensure prompt review by clinicians.
- Data entry of new and temporary registrations and relevant patient information.
- Scan patient information into computerized records.
- Deal with referrals to primary and secondary care.
