About the Role

In this challenging yet highly rewarding role, you will provide personalized care and support to individuals and families in their own homes. Your primary focus will be on enabling service users to live safely and comfortably in their communities, promoting their independence and well-being at all times.

You will work closely with our multidisciplinary team to assess client needs and develop tailored care plans that meet those needs. You will also be responsible for delivering care and support services, including personal care, medication management, and domestic assistance.


Requirements
  • A Level 2 Diploma in Health and Social Care or equivalent qualification
  • Experience of working in a care setting, preferably with vulnerable adults or children
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with clients and colleagues alike
