We are seeking a highly skilled and compassionate District Nurse to join our Specialist Dementia and Frailty Team. As a key member of our team, you will provide high-quality, evidence-based care to patients with dementia and frailty, working closely with our Advanced Nurse Practitioner to assess, plan, implement, and evaluate packages of care.

You will have responsibility for the day-to-day supervision and management of junior team members, providing guidance and support to ensure the delivery of excellent patient care.

Key Responsibilities
  • Conduct comprehensive assessments and reviews of care for patients with dementia and frailty
  • Develop and implement individualized care plans, working closely with patients, families, and other healthcare professionals
  • Provide supervision and guidance to junior team members, promoting their professional development and well-being
  • Participate in multidisciplinary team meetings and contribute to the development of care pathways and policies
Requirements
  • Registered Mental Health Nurse (RMN) qualification
  • Experience in community care and dementia care
  • Strong assessment and planning skills, with the ability to work autonomously and as part of a team
  • Excellent communication and interpersonal skills, with the ability to work with patients, families, and other healthcare professionals
