About This Job
We are looking for a skilled and compassionate adult support worker leader to join our team. As a Community Housing Support Worker, you will play a crucial role in supporting adults with learning disabilities and associated health issues and/or physical disabilities to live fulfilling lives.

**Estimated Salary**: £28,500 - £33,500 per annum based on 38 hours per week.

Job Overview
This is a challenging yet rewarding role, requiring a high level of empathy, understanding, and patience. You will work closely with our team to deliver person-centred support, enabling individuals to participate in a wide range of activities and reach their full potential.

**Responsibilities:**
* Deliver person-centred support to individuals with learning disabilities and associated health issues and/or physical disabilities, promoting independence and self-confidence.
* Collaborate with colleagues, managers, and other stakeholders to develop and implement person-centred support plans, ensuring seamless delivery of services and high-quality outcomes.
* Provide emotional support and guidance to service users, addressing their individual needs and concerns.

**Requirements:**
* A relevant qualification in social care or a related field (e.g., HNC/HND Social Care, SVQ3/4 Social Care).
* Experience of working with adults with learning disabilities and associated health issues and/or physical disabilities is essential.
* Excellent communication and interpersonal skills, with the ability to build strong relationships with service users, colleagues, and other stakeholders.
* Ability to work flexibly, including evenings, weekends, and sleepovers, and travel within the city and surrounding areas.
