About the Role
We are seeking a skilled Registered General Nurse to join our Community Team in Western Cheshire. As a Community Nurse, you will be responsible for delivering high-quality nursing services to patients in their own homes, working closely with other professionals to provide seamless care.
Main Responsibilities
Key duties will include undertaking holistic assessments of patients' health needs, coordinating care plans, and working collaboratively with other healthcare professionals to achieve the best possible outcomes.
Requirements
To be successful in this role, you will need to demonstrate excellent communication and interpersonal skills, with the ability to work effectively in a fast-paced environment. You will also need to have a strong understanding of community nursing principles and practices.
