About the Role
Quay Primary Healthcare CIC is seeking a highly skilled and dedicated Primary Care Physician to join our team. As a Primary Care Physician, you will be responsible for providing high-quality patient care, working closely with our team to ensure the best possible outcomes.
Key Responsibilities
- Assess and examine patients, diagnose, plan, treat, and review/evaluate their conditions.
- Diagnose and manage patients presenting with both acute and chronic conditions based on a management plan that includes a medication plan and other interventions.
- Identify health priorities and make timely and appropriate interventions to support patients presenting in an emergency and/or with complex/urgent needs.
- Assess and manage patients' physiological and psychological needs, including those with complex needs and multiple diagnoses.
- Arrange referrals for patients where appropriate via communication with the administrator on shift.
- Ensure that clear, accurate contemporaneous records are made of all patient encounters.
- Maintain own personal professional development and continuing medical education to ensure knowledge and skills are up to date.
