Job Title: Learning Disability Nurse Practitioner
Job Description:
This is an exciting opportunity to join Devon Partnership NHS Trust as a Learning Disability Nurse Practitioner. The role involves assessing eligibility criteria under the NHS National Framework for Continuing Healthcare and Funded Nursing Care, planning and commissioning packages of care, supporting individuals, their families, and health professionals through the Continuing Healthcare process.
The ideal candidate will have experience working with people who have learning disabilities, complex physical health, challenging behavior, and/or mental health needs. They will be skilled at imparting complex and sensitive information to service users and families/carers, primary care teams, external agencies, and safeguarding teams.
The salary for this position is £38,890 - £44,606 per annum, depending on experience.
