Health Care Assistant

About the Role
We are seeking a highly motivated and enthusiastic Health Care Assistant to join our District Nursing Service in the Conwy locality. As a key member of our team, you will play a vital role in supporting our district nursing team to deliver high-quality care to patients in their own homes.
Main Responsibilities
- Provide direct patient care as allocated by a trained nurse, following appropriate training and within the framework of a plan of care implemented and reassessed by a registered nurse.
- Organise tasks in a flexible manner according to priority of needs, ensuring efficient use of resources.
- Assist and support patients in maintaining an acceptable quality of life, promoting independence and dignity.
- Maintain accurate and legible documentation in accordance with Health Board Policy, ensuring confidentiality at all times.
- Order supplies, manage stock, and check deliveries to ensure efficient use of resources.
- Assist with any clerical duties as required.
