Job Title: Community Health Outreach Specialist

Estimated Salary: $48,000 - $62,000 per year

This role involves serving as a Community Health Outreach Specialist, where you will assess grievances and satisfaction data to identify trends and areas for improvement.

Main Responsibilities:
  • Participate in training sessions to develop essential skills and knowledge.
  • Effectively operate the patient intake/management system.
  • Contribute to the development and improvement of the HHP patient intake and management system.
  • Monitor the HHP patient advocacy system for optimal performance.
  • Acquire knowledge on how to work with marginalized communities.
  • Familiarize yourself with insurance policies and procedures.
  • Deliver exceptional customer service consistently.
  • Comply with a set schedule.
  • Recognize and adhere to the HHP Confidentiality Policies and Procedures.
  • Regularly check-in with patient advocates.
  • Solve complex cases and ensure timely follow-ups.
  • Guide new team members in their training and development.
  • Act as a point of reference for patient advocates' queries.
Necessary Qualifications:
  • Completion of secondary education is mandatory.
  • A minimum of 5 years of experience in the healthcare sector is required.
  • Candidates must be above 18 years of age.
  • Preferably holding some form of higher education or technical certification.
  • Demonstrate adaptability in handling changing processes and computer systems.
  • Show competence in basic arithmetic calculations.
  • Exhibit impressive critical thinking abilities.
  • Have experience using G Suite applications, especially Google Docs and Sheets.
  • Display superior communication skills.
  • Show the capacity to handle multiple tasks simultaneously and thrive in a fast-paced environment.
