Job Title: Community Health Specialist

Job Description:

We are seeking an experienced Community Health Specialist to join our team in Aberdeen. As a Community Health Specialist, you will be responsible for delivering high-quality nursing care to patients in their own homes. You will work as part of a multi-disciplinary team, offering one-to-one care and building lasting relationships with patients, stakeholders, and patient support networks.

Key Responsibilities:

* Cover the Aberdeen region, travelling to patients' homes
* Work as part of a multi-disciplinary team offering one-to-one care
* Deliver effective and high-quality nursing care to facilitate early supported transfer from hospital or prevent unnecessary admission
* Build lasting relationships with patients, stakeholders, and patient support networks
* Accurately document all patient care and clinical observations

Package and Benefits:

The package for the Community Health Specialist role includes:

* Annual salary of £33,000 - £38,000, negotiable on experience
* Company car or an extra £4,900 per year for car allowance
* Mileage expenses
* 33 days holiday including bank holidays
* Pension scheme
* Death in Service benefit (4 x annual salary)
* NMC fees paid
* Ongoing training and development opportunities

Requirements:

To be successful as a Community Health Specialist, you must:

* Have a full UK driving licence
* Be experienced in the care of IV devices and administering IV therapies
* Have a valid NMC pin number
* Be a qualified RGN
* Homecare experience is desirable
