Main Duties- Establish and implement effective care pathways for patients with complex needs, ensuring seamless coordination and collaboration across clinical and organisational boundaries.
- Provide leadership and guidance to a small team of complex care advisors, ensuring they have the necessary skills and support to deliver high-quality care.
- Develop and maintain strong relationships with stakeholders, including clinicians, managers, and patients, to ensure effective communication and collaboration.
- Identify and address recurring themes and areas for improvement in care pathways, implementing changes to enhance patient experience and outcomes.
- Collaborate with the multidisciplinary team to provide in-reach advisory caseload support and guidance, ensuring patients receive comprehensive and coordinated care.
We are looking for an experienced and skilled professional with a strong background in complex care and healthcare transformation. The ideal candidate will have:
