About the Role:
This position provides clinical pharmacy services to patients in both acute care and residential care settings. As a member of a multidisciplinary team, you will assess and recommend individualized treatments, act as the primary resource for drug information and education, and provide expert advice to healthcare professionals.
Key Responsibilities:
- Monitor drug prescribing, distribution, and therapy to ensure the safe, effective, rational, legal, and economic use of drugs.
- Maintain workload records and statistics.
- Practice patient-centered care by monitoring ongoing drug therapy.
- Communicate pertinent findings and recommendations to appropriate healthcare practitioners.
- Interpret medication orders, enter them into the computer database, dispense prescriptions, identify and resolve drug therapy problems, and consult with physicians on issues such as drug dosage, allergy, incompatibility, interaction, and efficacy.
