About the Role

We are seeking a dedicated and compassionate Support Worker to join our team in Playing Place. As a Support Worker, you will play a vital role in providing person-centred support to individuals with varying needs and interests.

Key Responsibilities

  • Empower individuals to live the lives they want to lead, promoting independence and autonomy.
  • Develop daily living skills, enabling individuals to participate in activities and engage with their communities.
  • Support individuals to access their communities for socialisation, employment, training, or friendships.
  • Contribute to, develop, and implement individual care plans, ensuring tailored support.
  • Communicate effectively and professionally with individuals, their family members, and external professionals.
  • Maintain the health needs of the individuals we support.
