As a Support Practitioner, you will work closely with individuals who have a wide range of support needs, providing them with practical and emotional support to help them achieve their personal outcomes.
**Key Responsibilities:**
- Provide individualized support to adults with complex needs, promoting their independence and well-being.
- Work collaboratively with individuals, their families, and other professionals to develop and implement personalized support plans.
- Assist individuals with daily living tasks, such as personal care, meal preparation, and household management.
- Encourage and support individuals to engage in meaningful activities, build relationships, and participate in their community.
- Monitor and report on individual progress, identifying areas for improvement and developing strategies to address these.
