Job Overview:

We are seeking an experienced and compassionate Healthcare Educator to join our Community Heart Failure Service in North Staffordshire. As a key member of our team, you will play a vital role in delivering high-quality patient education and support services.

Job Summary:
As a Patient Educator, you will be responsible for providing tailored heart failure education and information packages to patients who are newly diagnosed with this condition. Your primary goal will be to empower patients to manage their health and wellbeing needs optimally, promoting independence and self-care. You will work closely with the community heart failure specialist nurses and wider community healthcare teams to deliver this service.

Main Responsibilities:
  1. Develop and deliver comprehensive heart failure education packages to patients, incorporating virtual monitoring mechanisms and Flo telehealth digital technology.
  2. Conduct one-to-one sessions with patients, addressing their specific needs and concerns, and providing ongoing support and guidance.
  3. Work collaboratively with the clinical specialist team to evaluate and improve heart failure education delivery, incorporating innovative changes and group education sessions as necessary.
  4. Support patients in making lifestyle changes, advocating for their needs within the wider team, and promoting holistic care.
  5. Maintain accurate records and contribute to quality improvement initiatives, ensuring exceptional patient outcomes.

About You:
You will possess a strong educational background, excellent communication and interpersonal skills, and the ability to work effectively in a dynamic environment. A passion for empowering patients to take control of their health and wellbeing is essential. Experience in a similar role or relevant field would be advantageous, but not mandatory.
