Job Summary
The Central Surgery is seeking a highly skilled and motivated Advanced Clinical Practitioner to join our Primary Care Network. As an autonomous healthcare professional, you will work independently to assess, diagnose, and treat patients with a range of acute, non-acute, and chronic medical conditions.
Main Responsibilities
- Work autonomously and be accountable for your professional actions.
- Undertake clinical practice at an advanced level using expert knowledge and clinical skills to deliver holistic care to people accessing primary care services.
- Assess undifferentiated patients with a range of acute, non-acute, and chronic medical conditions, taking history, making appropriate physical examinations, formulating differential diagnoses, and carrying through management treatment plans.
- Discuss with registered GPs and, where appropriate, make referrals to other health professionals in primary and secondary care.
- Arrange hospital admission for appropriate cases.
- Undertake telephone triage work, telephone advice calls, and online consultations.
- Manage and undertake home visits, including Care Home visits.
- Manage Palliative Care patients.
- Prescribe medication as necessary as a non-medical prescriber.
- Maintain accurate record keeping in SystmOne/EMIS using templates and READ codes.
- Undertake regular medication reviews and medicines management.
- Request and interpret tests appropriate to patients' needs.
- Manage communications from hospitals and other providers, acting on requests for further action, medication, etc.
Person Specification
- First Level Nurse (RGN)/Qualified Paramedic.
- Post Graduate diploma or degree in Advanced Clinical Practice.
- Independent non-medical prescriber.
