Nurse Assessor Lead - Office hours at home, then travel for face-to-face assessments.

Salary: £42,000 per annum (dependent on experience)

Job Purpose

You will lead on clinical assessments, ensuring the delivery of individualised support packages. Working closely with the Assessment and Contracts Manager, you will ensure packages meet our model of support across an agreed geographical area.

Main Responsibilities:
  • Completing desk-based and face-to-face assessments of referred individuals for support.
  • Ensuring an APIE approach: Assessing, Planning, Implementing, and Evaluating.
  • Reviewing reports and evidence outcome measures to inform clinical practice, business performance, and quality measures. Maintaining continuous professional development and producing your CPD folder upon request.
  • Able to identify and mitigate complex risks for a community setting.
  • Reviewing pre-admission assessments in a timely and professional manner.
  • Producing clinically informed assessments with robust risk assessment and identifying risk mitigation measures.
  • Identifying measures to enable individuals through positive support.
  • Attending internal and external MDT meetings to inform assessments and risk mitigation.
  • Building professional working relationships.
  • Effective time management.
  • Record keeping: maintaining accurate, safe, and contemporaneous records.
  • Completing assessments based on current information, evidence-based, and identifying risk mitigation.
  • Identifying which completed assessments can be progressed to Business Development for presentation to Commissioners as they fit within our model of support.
Requirements:
  • Registered Mental Health Nurse or Occupational Therapist (Essential).
  • Experience assessing and working with individuals with complex needs around mental health, learning disability, ABI, personality disorder, autism, and forensic backgrounds.
  • Understanding of continuing healthcare or transition from hospital/residential settings into the community and a collaborative approach to issues associated with this.
  • Aplied knowledge of the Mental Health Act, CTO's, and the Mental Capacity Act, Deprivation of Liberty Safeguards, and Best Interest Decision making.
  • An expert in assessment and management of risk, innovative thinker, and natural communicator with strong report writing skills and computer literacy, and ability to work under pressure.
  • National travel and overnight hotel stays may be required.
  • Full UK driving license and access to a vehicle.
About You

As a qualified nurse or occupational therapist, you will have experience working with individuals with complex needs. You will be able to lead clinical assessments and produce individualised support packages. Your applied knowledge of relevant legislation and regulations will enable you to make informed decisions about risk management and best interest decision making.
