About the Role

We are seeking an experienced Advanced Nurse Practitioner to join our team at Rural Rother PCN. As an Advanced Nurse Practitioner, you will play a key role in providing high-quality, patient-centered care to our community.

Key Responsibilities
  • Evaluate patients with complex care needs using physical assessment skills and independent nurse prescribing (V300)
  • Create or update personalized care plans for vulnerable patients living in the local primary care network area
  • Triage and manage acute and non-complex medical needs of patients who are housebound, palliative care, and vulnerable
  • Work with other local service providers to ensure good communication between the multi-disciplinary team
  • Implement and evaluate individual specialized treatment plans for chronic disease patients
  • Identify, manage, and support patients at risk of developing long-term conditions
  • Provide advanced, specialist nursing care to patients as required
Requirements
  • Advanced Practice Qualification (Masters Level) or equivalent experience
  • Experience of working in a primary care/Urgent Care/A&E setting
  • Experience of working as a GP practice or relevant similar experience
  • Qualified Independent Nurse Prescriber V300
Desirable Qualifications
  • Qualified Triage assessment practitioner or other relevant experience
  • Minor Illness Qualification- or willingness to undertake training
  • Mentoring and supervision training undertaken
