Job Overview

The Directorate is excited to present a remarkable opportunity for a dedicated Advanced Clinical Specialist to join the One Coventry Integrated Team. This role is essential in supporting prehospital and consultant-led pathways, aiming to minimize unnecessary hospital admissions and enhance admission avoidance strategies.

The successful candidate will utilize advanced health assessment skills and will be encouraged to pursue non-medical prescribing qualifications. This position focuses on addressing a diverse range of medical conditions, ultimately aiming to improve patient outcomes and support individuals with complex healthcare needs in community environments.

Key Responsibilities

The post holder will be responsible for:

  1. Conducting thorough history taking, clinical evaluations, and implementing a variety of care interventions for patients, including those with undiagnosed issues.
  2. Employing specialized knowledge and clinical reasoning to plan and deliver interventions in collaboration with multidisciplinary team members across various settings, including patients' homes and community locations.
  3. Guiding patients to appropriate health, social, and third-sector services to reduce unnecessary emergency department visits and enhance overall quality of life.
  4. Facilitating change within the team by employing influential skills to support staff and contribute to service improvement initiatives.
  5. Managing caseloads effectively, determining when patients are ready for discharge or transition to other service providers.
  6. Triage referrals into the service, ensuring that care interventions are promptly implemented to meet patient needs.
  7. Assessing and mitigating risks associated with community care delivery.
  8. Participating in a 7-day working rota to ensure essential services are maintained.
