Job Title: Senior Social Worker for the Assessment and Safeguarding Team

As a Senior Social Worker in the Adult Assessment and Safeguarding Team, you will play a vital role in ensuring the safety and well-being of vulnerable adults. Your key responsibilities will include assessing individual needs, identifying risks, and developing tailored care plans in collaboration with multidisciplinary teams and individuals.

Key Responsibilities:

  • Conduct thorough assessments of vulnerable adults to identify their needs and risks
  • Develop and implement effective care plans to ensure the safety and well-being of individuals
  • Work closely with multidisciplinary teams, including healthcare professionals and family members, to provide comprehensive support
  • Identify and mitigate risks to vulnerable adults, ensuring their safety and well-being
