Job Summary
We are seeking a skilled Pharmacy Technician to support our team in delivering high-quality clinical outcomes in a supported environment. As a Pharmacy Technician, you will be responsible for providing exceptional care and services to our patients.
Main Duties of the Job
Your role will involve supporting our GPs and Clinical Pharmacist with medical reviews, CQC searches, and liaising with hospitals, pharmacies, and patients. You will also be responsible for assisting with medicines reconciliation, providing medication reviews, and supporting the practice-based pharmacist in delivering medicines optimisation and management incentive schemes.

Job Responsibilities
- Support the pharmacist with medicines reconciliation in accordance with practice protocols and update patients' medical records/prescribing accordingly.
- Support the practice-based pharmacist to provide medication reviews and Structure Medication reviews.
- Support medication reviews and medicines reconciliation for new care home patients and synchronise medicines for patient transfers between care settings and link with local community pharmacists.
- Assist in the delivery of medicines optimisation and management incentive schemes and patient safety audits in line with CQC search criteria.
- Support the PCN and practice to deliver on DES specifications, QOF, and other locally commissioned enhanced services.
- Duties in line with scope of practice to support demand and rapid change of general practice.
Person Specification
Qualifications
- BTEC/NVQ Level 3 or equivalent in Pharmaceutical Sciences.
- Registered with the General Pharmaceutical Council.
- Demonstrable commitment to professional development.
Experience
- Experience of Microsoft Office software.
- Advanced numeracy skills to GCSE Level C or above.
- Excellent keyboard and computer skills, including use of Microsoft Office.
- Demonstrable excellent communication skills.
- An understanding, acceptance, and adherence to the need for strict confidentiality.
- Ability to use own judgement, resourcefulness, and common sense.
- Willingness to be trained and developed.
- Experience of dealing with the public/patients.
- Ability to work at part of an integrated multi-skilled team.
- Take responsibility for, and accept outcomes of, your own decisions.
- Able to work under pressure.
