About the Role:

We are seeking a compassionate and results-driven individual to join our team as an Independent Living Coordinator. The successful candidate will be responsible for:

  1. Developing and implementing support plans tailored to each residents needs
  2. Collaborating with healthcare professionals, social workers, and other stakeholders to ensure seamless service delivery
  3. Conducting regular reviews and assessments to identify areas for improvement
  4. Maintaining accurate records and reporting on progress

About You:

We are looking for a highly empathetic and organized professional who can build strong relationships with residents, families, and colleagues. A background in social care, healthcare, or a related field is preferred, but not essential. If you possess excellent communication skills, a patient and supportive nature, and a commitment to delivering exceptional care, we invite you to apply.

The Salary for this position is approximately £26,707 per annum, depending on experience and qualifications.
