About the Role

We are seeking an experienced and motivated Support Worker to join our team at a leading provider of support services for individuals with a variety of needs. As a Support Worker, you will play a key role in delivering high-quality care and support to our clients.

Key Responsibilities
  • Working as part of a team of support workers to deliver person-centred care and support
  • Coordinating and allocating work tasks to ensure effective support delivery and adherence to individual care plans
  • Providing hands-on support to individuals with limited mobility, dementia, and other elderly care needs
  • Undertaking assessments and reviews of individual care plans to ensure optimal care delivery
  • Liaising with families, carers, and external professionals to ensure a coordinated approach to care
  • Monitoring and reporting on the progress and well-being of clients
  • Supporting with staff training, development, and supervision to ensure a skilled and motivated team
Requirements
  • Previous experience working in a similar role within the social care sector
  • NVQ Level 3 in Health and Social Care or equivalent
  • Experience of working with individuals in a care home setting
  • Strong leadership and management skills to lead and support a team
  • Excellent communication and interpersonal skills to work effectively with clients, families, and colleagues
  • A commitment to delivering high-quality, person-centred care and support
