Adult Care Practitioner

This Experienced Social Services Professional will be responsible for maintaining accurate records, contributing to local knowledge, and advising colleagues on legal and procedural requirements.
The estimated salary for this role is £45,495 per annum.
Responsibilities:- Record information and activities accurately and timely within the social care database, adhering to work targets.
- Contribute to knowledge sharing within the team and service about universal services, including those provided by the voluntary and community sector.
- Advise and guide colleagues in meeting requirements.
- Identify adults at risk and take action according to the Department's safeguarding policies and procedures.
- Conduct investigations and make recommendations for Adult Protection Plans.
- Adhere to council and departmental policies and procedures.
- Engage in supervision and appraisal through self-evaluation.
- Provide statutory support to adults, carers, and families.
- Collaborate with partners, including healthcare providers and voluntary sector organizations.
- Monitor and assess risk when working with families.
- A Social Work Qualification or equivalent is required.
