About the Role

We are seeking a highly motivated and dynamic individual to join our Torfaen Adult Mental Health Social Work Team within the Social Care and Housing Adults Services Division.

Key Responsibilities
  • Assess and plan care for adults with mental health needs
  • Work collaboratively with multi-disciplinary teams to deliver effective care and support
  • Develop and maintain relationships with service users, families, and carers
  • Contribute to the development and implementation of care plans
  • Monitor and evaluate service delivery to ensure high-quality care
Requirements
  • Registered Social Worker with a DipSW/CQSW/CSS/Degree in Social Work
  • Valid Social Care Wales Registration
  • Experience of care and treatment planning, monitoring, and evaluating service delivery
  • Ability to work effectively as part of a team and contribute to a supportive environment
Desirable Qualifications
  • Approved Mental Health Professional (AMHP)
  • Accredited Best Interest Assessor
