Clinical Team Lead

As a Practice Nurse, you will be responsible for the effective management of patients with long-term conditions, utilizing evidence-based practice and care for elderly and housebound patients. You will work as a senior member of the practice multidisciplinary team, ensuring nursing services are delivered effectively to the entitled patient population. Your key areas of responsibility will include infection prevention and control, health promotion, chronic disease management, and supporting the practice management team in reviewing and delivering clinical policy and procedure.
Key Skills and Qualifications:
Essential skills and qualifications for this role include a strong nursing background, excellent communication and leadership skills, and the ability to work effectively in a team environment. You will also have a deep understanding of clinical policy and procedure, as well as the ability to implement and maintain high standards of patient care.
